When you become a patient at HOPES, you gain access to a range of comprehensive services in one central location. Our community health center welcomes everyone, with or without health insurance. To get started, call us Monday – Friday at (775) 786-4673.

Scheduling Your First Appointment

Adult Patients:

If you are a new patient, we can assist you in scheduling your first appointment, often within the same week. Please note that your appointment will be scheduled based on availability.

To schedule your first appointment:

Call (775) 786-4673, Monday–Friday, and ask to schedule a new patient appointment. An Intake & Eligibility Specialist will take your information and call you back to schedule your first appointment.

Please note: After you are established as a HOPES patient, you can call anytime during normal business hours to schedule your regular appointments.

Pediatric Patients:

Establishing Care for an individual under the age of 18 is different than becoming a new patient as an adult:

  1. Call (775) 786-4673 and dial option 1 between 8:00 am – 5:00 pm, Monday–Friday, and ask to schedule a new pediatric patient appointment.
  2. An Intake & Eligibility Specialist will take your information and schedule your child’s first appointment. Pediatric new patient appointments can generally be scheduled within a week of your phone call.

Please note: After you are established as a HOPES patient, you can call anytime during normal business hours to schedule your regular appointments.

What To Bring To Your First Appointment

To ensure a smooth transition of services to HOPES, please bring these items to your first appointment:

  • The most recent list of the medications you are taking.
  • Medical records from either the hospital, your last emergency room visit or previous physician.
  • Completed HOPES intake forms: Patient Registration forms and Health History forms (These forms are available on the right panel and at HOPES).

Opioid Prescription Policy:

Northern Nevada HOPES does not prescribe opioid medications to its patients. If you are currently taking opioids, our providers will explore alternatives with you.

Establishing HIV Care:
