Temporary and Interim Position: Part-Time HIV /Retention of Care Liaison
Are you looking for an opportunity to cross train and learn more about HOPES? We have an interim position available for the next 3-6 months that will offer you an opportunity to be involved on the frontline of providing our HIV clients with intensive case management services! This new role will be partnering with our existing HIV Retention of Care Liaison half time (50%) to job share some of the daily responsibilities and remaining in your existing role 50% of the time. This position includes gaining knowledge and active participation of our organization’s commitment to being a Patient Centered Medical Home (PCMH) and engaging in activities that enhance these patients care delivery. This role is vital to HOPES and provides direct assistance to our HIV clients in helping them gain access to services, retention and coordination of care, oversight of the patient’s medical case coordination and linkage to appropriate services. A primary focus is to identify clients currently out of HIV care, and to reengage these patients back into some level of treatment again.
The Medical Case Management currently includes:
*Medical Case Management does not include the direct delivery of medical, clinical or other direct professional services. Medical Case Management is on behalf of the patient, not to the patient and is management of the plan of care, not the patient.
