Health Centers Are the Place for Wrap-Around Care

We recently joined heath centers across the country to celebrate National Health Center Week! We’re grateful for our generous employees, board members, individuals and organizations who contribute to helping us reach our vision to create a healthier community. You all make HOPES a special place!

Health centers like HOPES work to reduce barriers to accessing care. We offer services like transportation, an onsite pharmacy and labs, and case management. Case management is essential to the wellness of so many of our patients. Our case managers work closely with our medical and behavioral health providers to identify patients who need services like job training, housing support, resource navigation and more. Our case managers meet our clients wherever they’re at and work with them on a plan toward wellness. Last year, HOPES’ case managers engaged in over 4,500 appointments to support our patients.
