Are you ready to change the world?  Our Community Health Worker will engage in education and outreach to promote the teen pregnancy prevention program in accordance with the grant requirements and federal, state, and local laws and regulations. You’ll work with teens, families and community groups to encourage those discussions necessary to help in the prevention of unplanned pregnancies.

Northern Nevada HOPES is a community health and wellness center serving the Northern Nevada area.  We are a non-profit community health center located in a historic and quaint area of downtown Reno, Nevada.   • We are a National Health Service Corp site, and our providers are eligible to apply toward a federal loan forgiveness program

The Community Health Worker’s objective is to support the core goals of the Teen Pregnancy Prevention and Families Talking Together programs. You’ll engage in education and outreach, participant recruitment, community networking, family and group presentations and many other duties to meet the goals and objectives of this grant. HOPES has an integrated team model approach to patient care. Part of the CHW’s responsibilities will include participation in organization wide Patient Centered Medical Home activities to enhance patient care. This individual is also responsible for being an active participant in Continuous Quality Improvement activities.

We’re looking for:

  • Someone who loves to be challenged, because you will be daily.
  • Patience is not just a virtue here, it is a vital skill.
  • Know, learn, and keep up to date with the Trauma Informed Care, Motivational Interviewing and Harm Reduction philosophy.
  • Experience working with youth, families, schools, and/or community organizations; knowledge of HIPAA is super-duper!
  • Comfortable candidly discussing sex, sexuality, sex work, prevention of pregnancy, and abstinence.
  • MUST BE BILINGUAL!
  • Able to work in a fast paced environment, self-directed, motivated but with ability to work independently as well as part of a team.
  • MUST be free of judgement and willing to accept choices of others.
  • Computer skills essential for conducting data collection, entry and analysis.
  • Authenticity-Be you, the genuine you will ring true with our clients.

  • Love to learn something new every day.
  • Bachelor’s degree or formal studies in nursing, public health, human development, psychology, social work OR equivalent practical work experience preferred.
  • Well-developed presentation skills and the ability to present ideas to diverse audiences.
  • Perform a broad range of simultaneous tasks, we may be pulled different directions at the same time, personal skill to identify and prioritize is also vital.
  • Be nice; for some people you will interact with you may be the only person who is.
