New Client Information

As part of an integrated care services delivery system, our multi-disciplinary team of doctors and nurses, work closely with our social workers, case managers and health educators to assist our clients live longer, healthier lives.

 

Individuals wanting to become HOPES client/patient must:

  • Bring written proof of their HIV + status to their initial orientation appointment.
  • Must provide a copy of a positive Western Blot or ELISA test for HIV.
  • Clients may also provide a letter of diagnosis from their physician and/or medical records that show documentation of HIV + status.
  • To receive Social Services you must be a resident of our Service Delivery Area (All of Nevada with the exception of Clark, Nye, Lincoln and Esmerelda Counties )

 

The orientation appointment includes:

  • Assessment of client service needs.
  • Completion of required paperwork; including a medical history.
  • Completion of required laboratory tests which are done on-site and scheduling of appropriate appointments as requested by the client or as needed to provide client with the best care.
  • Referrals are made if necessary to connect a client with medical and medication insurance.
